You generally need official proof of the family tie (such as a marriage or birth certificate) plus evidence of shared accommodation and financial coverage of the stay.
Because the permit is based on the family relationship, you must document it with official certificates, for example a marriage certificate for a spouse or a birth certificate for a child, usually apostilled or superlegalised and officially translated into Slovak.
Applications also commonly require proof of common accommodation with the sponsor and proof that the stay is financially covered, with the sponsor often acting as the guarantor. A medical certificate confirming you do not have a disease threatening public health is typically required around the time you collect the residence card. Gather and translate these documents early to avoid delays.
