These include being of Polish origin, holding a valid Karta Polaka (Pole's Card), being the spouse of a Polish citizen with the required marriage and residence, or meeting another qualifying continuous-residence route. You will generally need a valid travel document, biometric photographs, and route-specific evidence — for example proof of Polish ancestry, the Karta Polaka itself, or marriage and residence documents. The application is made to the voivode at the urząd wojewódzki.
