If your contract is for four years or more the permit is normally issued for four years and can then be extended; for shorter contracts it covers the employment period plus an automatic job-seeking extension, and it can never run beyond your passport validity.
The duration is matched to your employment. Long contracts get up to a four-year permit, while shorter contracts get a permit covering the job plus a short job-seeking buffer at the end.
To extend, you apply before the current permit expires, and you should make sure you still meet the salary and Danish-standards conditions - including the requirement that your salary is paid into a Danish bank account. Your passport must also remain valid, since the permit cannot extend past it.
