Slovakia's EU Blue Card, also under Act No. 404/2011, is designed for non-EU professionals taking up highly qualified work. To qualify you generally need an employment contract for highly qualified work of at least six months, a recognised higher-education qualification (or, for certain IT and communications roles, equivalent skills), and an agreed salary that reaches the statutory multiple of the average gross wage in Slovakia.
As with other residence routes, you also provide health insurance, proof of accommodation, a clean criminal record and a valid travel document. The card is normally issued for up to five years, or for the duration of the contract plus a buffer period, which makes it more stable than a standard employment permit.
