The key conditions are that you have a stable and regular source of income to support yourself and any dependants, and that the business meets a viability test — for example a minimum income, employing a set number of staff, or evidence that it will meet these. You will also need health insurance and proof of accommodation, plus company and registration documents (a KRS extract or CEIDG entry) and proof of no tax or social-security arrears. The application is made to the voivode at the urząd wojewódzki.
