No, you cannot extend it and it does not lead directly to settlement, but you can switch to another visa such as the Skilled Worker visa.
The Graduate visa is a one-time, non-extendable route, and time on it does not count towards indefinite leave to remain on its own.
To stay beyond it, you switch to another visa - most commonly the Skilled Worker visa once you have a qualifying job offer from a licensed sponsor. Plan the switch well before your Graduate visa expires, because falling out of status can cause serious problems.
