Yes. The status can be revoked after twelve consecutive months outside EU territory, or after six years of absence from Italy, among other reasons.
The long-term permit is durable but not unconditional. It can be lost if you are absent from EU territory for twelve consecutive months, if you are absent from Italy for six years, if you acquire an equivalent long-term status in another EU country, or if it was obtained fraudulently.
It can also be revoked on serious public-order or national-security grounds. If you expect long periods abroad, check the rules in advance and keep evidence of your ties to Italy, because once the status is lost you would generally have to rebuild the five-year residence again.
