Changing to a new employer requires a new application - your permit ties you to the specific employer and job stated on it - although you can change duties within the same company if the salary and conditions still meet the rules.
Your permit authorises you to work only for the named employer in the role described. If you move to a different company, you must submit a fresh application (and the new employer must also be certified to use the Fast-track Scheme).
Movement inside the same company is more flexible. A change of duties, a promotion or a relocation within the same employer generally does not require a new permit, as long as your salary and terms continue to meet the scheme's standards.
